You don't need an either/or on this one. Anyone who is an American citizen 65+ qualifies for Medicare Part A and B. Part B has a premium. You don't have to take Part B, but it is important in paying for doctor visits, as well as testing.

Medicaid is for low-income people and can be used together with Medicare. There is no better between the two.
