My family is moving from Gilroy, California, to Carey, North Carolina.
I need someone to head up the details after discussing with me first about how to go about moving him from his facility now in CA to a place I am hoping the manager can find for us who will actually do what they say and respond to my requests.
He has vascular dementia, but he is still highly functional. While he is, I would like to move him so we can get the right care for the right price.
No government agency will be an advocate for me and it has been a daunting and frustrating task for me over the past two years since we moved him from home to this terrible establishment.

I am looking for a geriatric care manager to coordinate his move. And take over for me, as this has become so traumatic. I cannot do what I need to for myself and my family.
I will be paying for his care, but I would like to find a place that I can trust and that will actually give him the services he needs not just what they want to charge.

I'm concerned that you will be paying for his care. This is not a good long-term strategy and may delay or disqualify him from Medicaid should he need it (and many elders do). Also, the cost of care increases as his needs increase, and can be $5K and more per month. Your dad is "only" 77 and sounds like he has no other health issues beyond cognitive. Unless you have very robust financial resources, you could be paying thousands per month for many years. I am PoA for my 2 aunts, who are 100 and 102 yrs old. They still have no profound medical problems beyond cognitive so could live many more years.
After you secure a Geriatric Care Manager, I would also consult with an elder law/estate planning attorney and Medicaid Planner for NC, where you will all be relocating. Do not fear Medicaid. My MIL is in a very fine facility as a Medicaid recipient. She is 87 with no health issues, just cognitive and refuses to get out of bed so she is in LTC. Medicaid is run by each individual state so their rules can vary and that's why you need to talk to an expert for your new home state. Most state's Medicaid financial "look-back" period on the application is 5 years...and this is why you must be very careful how you manage your father's financial affairs and do not co-mingle your funds with his, or do anything that looks like gifting money to yourself or to him or to others from him. Consult with the Planner.
Since your dad is a vet, have you contacted the VA for options? Geriatric Care Managing is a newer field, and there is a huge demand now that Baby Boomers are aging and there is labor shortage. If you have trouble finding one, you could consider seeking to hire a retired Social Worker in NC to help -- they will probably know as much as a GCM. I wish you success in your move!
