He is acting confused with bathroom issues. He wears depends, but takes them off sometimes or misses the toilet. Has little or no speech has had mini strokes and has been diagnosed dementia.

That's a good article. I'd also discuss it with his doctor as a change in mental status should be reported to this doctor. I'd rule out things like UTI, other infections, meds, etc. But, if it is the dementia, I'd read about how others have dealt with it. Some people just constantly supervise the patient, so they aren't allowed to go into the bathroom unsupervised. And there are also adult onsies that snap in the back, so the person cannot remove them. They keep the patient from removing their Depends during the night. I think those items can be found available online.
