My son, in Archbold, Ohio, is 46 y/o, has end-stage renal disease, and is severely hampered by Obsessive Compulsive Disorder. Needs one-on-one input for basic living skills (not a full-time deal).

Hi, Alcwert. I'm bumping your question up as it seems to have been lost in the shuffle. Hopefully others might have some ideas.

I see from your profile that your son is enrolled in a state program but not really seeing any benefit. Just to brainstorm here, I wonder if there might be any help available from a kidney disease organization, or maybe a religious charity in your area (in my area Jewish Family and Children's Services is wonderful, and helps people of all backgrounds). Any mental health charities in your area? Any "friendly visitor" volunteer programs in your county that your son could be enrolled in to get a little socialization? You both definitely need and deserve support.

I am so sorry about your son's illness.

There are agencies that provide the services you mentioned that you could hire for the number of hours needed.
You might also try Area Agency on Aging to see if there are services available that would benefit your son.

The adult day care in our area run by the Area Agency on Aging provides transportation to/from home, basic care/supervision, meals and a program of activities each day. The age range of participants is 34-93 but it works well because although everyone has some challenge, very few have the same difficulties. If someone doesn't want to participate in an activity, that's no big deal.

Have you tried your County or State Disabilities Dept? I would start with the County. In my town we have ""Resources for Independent Living". The Disabilities Dept should be able to help you find a resource.

I just looked up the Ohio Medicaid SRS program. It says that your son should have a Manager who helps coordinate services for him. But, your son needs to do his part too. If he isn't happy with the provider he can change at time of open enrollment. I would think if he is not happy with the Manager, he has the right to request another one.

I was able to download the booklet describing the Manager's responsibilities.
