After your surgery, you will probably have some Physical & occupational therapy in the hospital before discharge. Tell them you have no one to take you to outpatient PT - will need transportation or you will need to be admitted to rehab facility for inpatient rehabilitation.
Some outpatient rehabs do have transportation. Usually the bigger ones located or affiliated at or on hospital grounds. My mom went to outpatient rehab (they also had an acute care rehab on site - and affiliated with the hospital) but moms outpatient was for anyone and everyone - knee replacement - strokes etc etc and they had transports coming and going all day so check a few local outpatient rehabs that are connected with hospitals (but maybe smaller places do this as well).
VBviets, if your location was in your profile, we could check to find local transit organizations. What you want to look for is either a transit company that has small bus or point-to-point services. These aren't the full size linehaul buses, but ones that are more the size of some commercial vans. In my area, they're generally equipped with a platform on which a wheelchair can be raised to the level of the bus.

You may have to do what a friend of mine who lives alone did after her knee replacement surgery and that is go directly into a rehab facility, where you will receive the PT that you need. I believe her insurance covered it, and it made her life much easier. Or you can do what gladimhere suggested and see if you can receive PT in your home, which I believe is usually an option as well. Were you offered the opportunity for recovery and PT/OT in a rehab facility? That's an excellent way to recover.
