I recently had this question for myself.
The hospital said I could not use a service like Uber or Lyft as the drivers are not vetted and the hospital could not take the chance something would happen.
Check with your local Senior Center or Senior Volunteer service often they will have volunteers that will drive someone to a doctors appointment and pick them up. There may be a fee if you are out of the area (live in one area but the hospital treating is in another area)
Some Public Transportation will also do pick up and drop off for disabled and seniors with a 24 hour notice.
In most cases it is best to find someone that can drive you and pick you up.

(if you want to be real sneaky and you think you can be ok to drive drive yourself to the hospital and then have a friend pick you up and drop you off at your car. BUT since you will probably be under the effects of pain killers and anesthesia this is not a good idea and your insurance would probably be cancelled if something were to happen even if it was not your fault)

Have done this a few times. Have used a Taxi to and from the ER. And also a private transportation service for wheelchair transport.

If you ask the hospital/facility in advance, they'll be happy to tell you which companies are used most often there and give you the phone number.

I'd think Uber, but I don't know if they'd want the liability. Plus you'd probably need help getting into the house and situated. Look at your extended family or even a kind neighbor.

Are you a church-goer? I know I could make one phone call and have 50 people offer to help me.

I've never heard of a surgeon's office offering something of this sort.

I wish you luck--this sounds challenging.

Have you asked your surgeon's office? I know that kind of info is usually available from our local offices. If it's not too great a distance a taxi or ride share service might be all you need.
