Yes. From what my RN daughter says, when u go AMA they are not entitled to do anything for you. No prescriptions, no discharge instructions, no setting up in home care or ordering equipment you may need. That is the consequence for going AMA. You r on ur own.
So now, you see you PCP and ask them to help with prescriptions, in home care and helping u order equipment.
The vast majority of hospital records are electronic. The PCP doing direct care might just need to look at the record. Particularly if practice is in the same group. Otherwise the patient fill out a form for access.
AMA. Against medical leave is what I think DrK is referring to. The patient (or their lawful representative) should be able to get a copy of any medical records. Especially if their insurance was billed. If the patient was taken to an alternate provider, that office would have a form the patient can sign, requesting info to be forwarded to the new provider. I have had the situation where the medical records were not IMMEDIATELY available upon leaving an ER with a relative. I had to return a few days later to get the records and they weren’t leaving AMA
Usually prescriptions are electronically sent to the patients listed drug store for processing.
Medical Equipment? Like a walker? Yes, I would expect the patient should be able to leave with any medical Equipment that the patient insurance covers the cost of.

With what you have written we can only guess what your problem is.
Who is withholding from whom?
What state/country is this in?
Is someone the MPoA for the patient? Or guardian?
Is the patient in hospice care?
Does the patient have a written Advance Healthcare Directive?
Is the patient incapacitated cognitively or consciously?
What does AMA stand for?
