And sign and date. There is no longer a lot of money in the medical industry to try to keep us alive, DNRs are usually respected with ease. There is no reason, if you question this, not to make out a new POA/advance directive for health care and have it witnessed or notarized according to the dictates of the State you live in. When my brother fell ill and was hospitalized a social worker came in and said "this advance directive is 20 years old; it is valid but we suggest making another on this date. If we give you forms you and Sister can do them, and we provide a list of volunteers who come to notarize". That's what we did.
