My mother doesn't like to go to physical therapy for her knee pain. She tries to come up with any excuse not to go. I have a feeling that sooner or later she'll quit going. If she does quit going, can her insurance company cancel her policy for refusal of treatment?

If she refuses PT three sessions in a row, the service will end, and noted as "failure to progress". This will not result in a policy cancellation.
