Where do I find info regarding insurance address and group plan number?

I recieved my bill from the hospital and on the back it asks for information regarding my insurance. I received emergency medicaid but I'm struggling to fill out the the paperwork on the back of the bill. It asks for "insurance company's address" and "group plan number". I don't see any of these in my paperwork and certainly not on my medicaid card. Any help would be much appreciated, thank you for all your help.

Not quite sure I follow you, but it sounds like Medicaid is supposed to pay for your hospital stay? I would just write in "Medicaid" as the primary insurance carrier ( i.e no "insurance company name") and then your Medicaid card #. Other option is to call the hospital billing dept and they will help you find the right info.
