I care for a gentlemen who has become incontinent. Yes, he wears depends.

The problem is his body shape. His belly is so huge and solid that he can't keep anything up. Including his pants. Therefore the depends hangs down from his crotch. The depends can't catch all the urine. I have tried everything I can think off to keep the depends in place. Nothing works! Therefore we have the washing machine going all the time.

Do what other people with big stomachs do - women often have big stomachs so they buy pants a size or two larger (the elastic around the waist is always too tight for comfort). Then cut about an inch into about three parts of the front of the elastic waist (elastic is now cut and it greatly widens the part that goes around the waist). But instead, pull the front cut up elastic portion of the pants and place it under the excess stomach on the front of the body. But pull the rear elastic part of the pants up in the back so it fits snugly around the waist. Works perfectly. Try this with depends - low waist over crotch under belly fat and pull rear waist as high as possible. Good luck.

leslie3: Imho, he can either try XL Depends (or larger sizes, e.g. 3X, 4X, 5X or 6X) or suspenders to hold up his pants including the men's undergarment.

I have total urinary incontinence. I wear disposable pullup briefs by Depends and Guards inside that catch the majority of the urine.
It's not a perfect solution, as I sometimes leak over the side of the Guards, but it is far better than external catheters and a leg bag. I quickly develop a rash on my scrotum and lots of burning pain when I try to use them. The adhesive seems to keep the skin moist too long and I have diaper rash.

Since the gentleman has a very round abdomen, I suggest

Suspenders - with all his pants. You can even hook the suspenders to his Depends while he is wearing clothing.

Condom catheter - it rolls over the penis and has an opening at the end to attach to either a leg bag or a large bag used when in bed. Make sure to get the leg strap to help support the weight of the bag.

Pouches - these work similar to the condom catheter with supposedly less pressure on the penis.

I am having good success with the high absorbency Depends for men with a liner of the high absorbency urine pads (ones for men or women) for overnight use. I also put down "puppy pads" on seats and bed so I end up doing less housework.

Try the Ultra Guard for men from Wal Mart or thicker ones from Amazon. It is like a big Maxi Pad but for men. Instead of changing diapers a couple of times a day, all I have to do is change the pad. At night, he has a diaper, the pad and then special briefs that I bought from Amazon. They are super thick and cost about $12.00 a piece. Well worth it.

Wrap his penis in a Depends, then put another Depends on him, the regular way.
I watched a CNA do that and it worked.

Condom Catheters. Get a penis measurement ring to get the right thickness size. He will also have to wear a leg bag during the day and a bedside urine bag at night. You also might need an optional extension tube. They actually work quite well, and are easy to get used to. Your urologist can start you with a supply and Medicare will cover it with a doctor’s order. Alternatively, you can buy some condom catheters and leg bags for a trial on eBay.

Have you tried depends with the Velcro type adjustable straps on the sides aka wrap arounds? You could also try adding a chucks pad to the front of his brief to help catch urine

Another thought...
If this is only bladder incontinence, why not look into an 'external catheter' (non invasive) that drains into a bag - less washing, more emptying.

I’m a small lady with a small jelly roll and things like stockings tend to roll down. So I think I know what you mean. If you can’t get a brand that will hug the crotch and waist, you will continue to experience leaks. I’m sorry to say but depends, McKesson, and other store brands did not work for us. Tranquility Premium Overnight is the brand that is working with urine.

Thank goodness he can still go to regular restroom for the other so don’t know how well they would contain for that and I’m not ready to find out. The matching booster insert did not work because for whatever crazy reason, he would pull it out the way and then use the boy’s room. Did not take me but finding one pulled out and thrown to the side. Thankfully it was unused but I discontinue ordering because I would not want him to pull a used one out and toss to the side.

The problem I’m having is he still wants to be in his 20’s and stand while trying to pull it out from the side or top which leads to wet floors and clothes. So if I’m not on him to require sitting, that’s when our accidents happens. When I see wet pants in the morning and the bed/bedside potty is dry, I know he has pulled the hose pipe out (either from top or side) and fallen back to sleep. Still a steam point for me.

But when he does not try to pull it out, that pull-up catches all the urine.

I love that old-fashioned suspender/bed sheet suspender idea. However, you may have to scour the internet for some that won't have metal parts. Otherwise see if smaller size underwear may work (a size down) or try different brands.

I used, for my Husband the pull-up briefs (Depends is a brand name) AND I also used his regular underwear over them. the tighter brief that he had worn previously help the pull-up in place better than just using the pull-up.
Try a pair of his tighter fitting normal underwear over the product you are using now.
The other thing you might want to look into is the size. Look closely at the waist size they give on the box not just the "letter size" (S,M,L,XL)

Are they pull up Depends or the kind with the flaps on the side that you pull out and over? My dad is a big guy. I finally found a brand Tena that works really well for him. He's a 2xl and they were life savers. I've know several people that have had to use a standard belt to hold them up as well.

Have you tried the old fashioned suspenders?
Just clip them on and adjust for a comfortable fit.
If there is any risk of injury (from metal parts, or too stretchy), you might fashion something similar to tie on and place over the shoulders - a bit a trial and error will be required.
