Not sure if its any different in other states but 6am to 7 am. It depends on the morning routine and the ratio of aides to residents. My daughter works in NHs. She said therapy was usually started after breakfast which is about 8am. Medicare felt that therapy should start at 8 am.

Mornings are hectic in a NH. Residents need to be up and dressed for 8 am breakfast. Then there are med passes. Maybe baths. Then 12 pm lunch.
