I'm taking care of a family friend for 4 years and want to know who to contact to see if I'm eligible for getting paid, as it's too hard to work full time and do it.

If you begin to work for a home-healthcare agency in order to care for your neighbor the family friend would have to sign a contract with that agency and then the friend would pay them who in turn would pay you. Additionally, you would probably be expected to work other jobs as well as an employee of an agency.

If your family friend has been receiving your services for free he/she is not likely to want to begin paying an agency thousands of dollars a month and by the same token if you've been caring for your friend for free it might be difficult to to get him/her to begin paying you after not paying you for 4 years.

You should be paid for your services. Unfortunately the time to have that conversation was 4 years ago. Has your friend's health declined and that's why it's become more difficult? If that's the case it would make a valid point.

That was my first question when I found this website 5.5 years ago. Usually the only option is if the person needing care is able to pay you. There are veteran and Medicaid benefits for that person if they are eligible but they would need to qualify. This s the most often asked question on AC, look around you may find other options.

If you type "how to get paid" into the site search you will discover that this is a very popular question with very few good answers.
If you want to be paid enough to actually have a decent salary that you can live on I think there are really only two options, draw up a caregiver contract between you and your neighbour and have them pay you OR some people take whatever training is necessary and ask to become employed by a local agency as her caregiver. If you decide to work privately there are good reasons to do it properly so that you are paying proper taxes and deductions, it protects you future pension and it also protects your neighbour from being accused of gifting away large sums of money if they ever need to apply for medicaid in the future.
