I'm looking for an assisted living for my mom (51) who is diabetic, has issues going to the bathroom and is a fall risk. Any advice?

Ben, the firat thing you should probably do is call your local Area Agency on aging and ask for a "needs assessment" for your mom.
You need to find out if her multple problems can be handled by an assisted living facility in your state or if she needs skilled nursing care.
