If paying family caregivers, do they need to pay taxes or can it be a gift?

I am POA and want to pay family members for providing care 24/7 as required. Do they need to pay taxes or can it be a gift? Worried about 5 year look back.

If there is not a care giver contract between the family members and the person being cared for, then your giving them money would be considered a gift and worry you about a 5 year look back.
