I want to pay out of pocket for home care services. What do I do?

Unless you are active in her care and can visually check on her regularly this doesn’t seem like a good idea to me. Just putting someone in her home is not always a good solution. JMHO

Try looking for an agency near your place first. Better to look for a home health care agency who has been in business for quite some time now.

Also see if they have RN's (registered Nurses) and LPNs (Licensed Practical Nurses) if you're going private nursing.

If you're looking for one in Philadelphia then I can recommend you to check Golden Health Services. They have been in business since 2001 and they offer services for 4 hours or more uptil 24 hours a day.

For "live in" will be more expensive. Some agencies will do "live in" but there are hours that must be followed. This you would have to discuss with the agency and how they do this.
I most certainly would not hire someone that is not from an agency for "live in" with all the problems I see in the news and read about trying to get someone out of your house that has "established residency" and going though the process of eviction is a nightmare.
I would also check your homeowners policy if you do not go through an agency to determine you level of liability if something should happen.

You contact an agency and interview them, they will also interview you. As a matter of fact contact several agencies and go through the process as to what you expect and what they expect.
You ask how much it will cost per hour, per day and if you have to sign a contract if so how long is the contract and how often they review the needs and level of care your loved one requires.
Then you will be billed for the services.
Insurance does not generally cover the cost of caregivers.
If you are asking how to find a caregiver that does not work for an agency so your costs will be a bit lower you can find caregivers on sites like Care.com as well as some postings in the paper.
Keep in mind that while an agency may cost a bit more they generally will cover workers comp so if a caregiver is injured while working in your house they will be covered by the agency. If the caregiver gets sick the agency will or should send another to cover that shift. If you or your loved one does not like the caregiver assigned you can ask for another, much easier than trying to find another through a paper or an independent site.
