Really, your putting that kind of milage on your car for a friend? Why can't his doctors's or whatever be closer. Does he have to go to this many appts? I found my Mom was going to her PCP every 2 months. She was on Cholesterol and B/P meds only. Even the nurse said something when I took her. I told the nurse "if he says, so why are you here, it will be the last time she comes unless sick". He did, she didn't go back again till she needed to.

Yes, your friend should be paying you for gas. He should
also find other transportation. If he gets Medicaid, they gave services. Counties usually have Senior bussing. You could then just be back up.

No, only if the miles were for you, your spouse or a dependent. Your friend should reimburse you for gas or time or both. Or, call social services for your friend's county to see if they qualify for medical transportation services, or contact the local Area Agency on Aging for resources.
