Talk with the Social Worker at your dialysis center. He/she would know of resources to help you out. In my experience, dialysis patients are discouraged from driving home because of the risk of their blood pressure dropping, feeling dizzy, and being tired after treatment. There must be services in your area that provide rides at a low cost. Best of luck!
Talk to your doctor's office and tell them you have no transportation. They maybe able to set you up with an agency. Office of the Aging usually has Senior transportation. Red cross may have some volunteers. My Church had volunteers.

Often there are Volunteer groups that will provide transportation to medical appointments for free. Ask at the doctors office or at the dialysis center if there is transportation available. And last but not least. Uber, Lyft, cab are all viable options for transportation.
