The fact that all this happened without your knowledge puzzles me, I'm not being judgmental here but did you burn out and dump him at the hospital and run? If that is what happened the State may have been awarded emergency guardianship, which supersedes your POAs.
The POA generally does have the ability to arrange for a different living situation for the person. But you probably need to develop a safe discharge plan. Start with the facility he is in. Ask them for a care plan. Arrange care. Discharge him.
You can also initiate a lateral transfer to a facility you like better. Just get a plan together and get him out of there safely.
