I believe I have dementia because forgetting has gotten much worse. I went to the wrong place for a Dr. App. Any advice?

I could not recall where the real appointment was. I am also sure I know the time of the appointment and get it wrong. Next day had no idea what went in a cubbard space that had the same dishes there for many years. Tonight was totally forgot to pick up a friend.
