How do we control that Grandma does not put her hands inside her pullups and touch her poo while sleeping? She has had couple of strokes. She is now on bed rest all the time. Her mind is not as it used to be, it is declining.

Maybe she is having a BM early in AM... Try checking her earlier in the morning, change her.
When you put her in bed for the evening, make sure she is clean. Get her up a bit earlier the next morning, check to see if she had a bm.
If yo can, have someone check her in the middle of the night.

This, is almost impossible to control... a home I had a person who would do just that... You cannot really tie their hands, or anything else.
Perhaps if you feed her earlier before bedtime, so hopefully she has a BM before sleep ..?
you can't control a body totally... Maybe encourage her to go before... like a laxative a few hours before bed. and hope for the best. Ducolax.. I think is the brand.
Try to get her regulated.. My relative place, feeds their residents around 4 pm, and bedtime a couple hours afterwards. Biggest meal is lunch... Light foods at night...
