There is access to a social worker at every US Dialysis center. Find out who it is for your mother & make an appt to meet with her and her nephrologist (her kidney doctor).
If she’s bedbound, how does she get to dialysis?
If she is ill and non responsive call 911. If she’s admitted, have the discharge coordinator place your mother in a SNF.

You have many many resources available. She may have to apply for Medicaid as that pays for nursing home long term care.
