Look at the state dept of public health website. You are supposed to maintain that certification on your own and follow all deadlines. You should receive a card and hold onto it. Usually it has a number on it. I have to maintain my licensure in whatever state I live. Let it expire then I cannot work. You can check online with your state on how to redo an application. You need that graduate certification so go back to the school. I hope it was accredited.
If your certification is like my medical liscense, I have to pay a fee every other year to keep it current and show I have kept my education up. Just look and see what agency you paid. Then look up thier website and there should be a link. Good luck!

They write to the board that certified you. You do not say if you are a CNA, a LPN/LVN or an RN. Look up your job description and certification board for your State. That will give address and phone number.
I am having trouble finding certification for Medical Assistant. I called the college they told me they have all medical exams taking through Kansas Department of aging but I cant seem to find the right spot to look it up and find my proof of certification for a jobI am tying to apply for.
