I would see if you can make an appointment at Social Services to speak to a caseworker. Moms caseworker did the application for me. Then gave me a list of things I needed to provide within the 90 day period allowed. You will need to know the basics. If they have a home, the market value, same with a car. SSN#. Any life insurance policies, bank accts, IRAs, CDs, bonds, stocks. The more you have at the time of the appt, the better.

The person must have no assets. If they have IRAs, bonds, stocks etc, they have to be liquidated and used to pay for their care.

The form can be found on your specific county's Department of of Health website. Usually it has its own navigation so should not be hard to find. I would print out 2 copies: one is the "scratch" (practice) copy and the other is the final.

Whoever the app is for, the applicant or representativce will need access to private information such as DOB, SSN, banking statements, etc. The form is "easier" if it is an unmarried person with only SS income and a little bit of cash in their bank account. Usually 1 house and 1 car are allowed.

Please know that once the app is submitted, if Medicaid has any questions or requests for more "proofs" they do it by snail mail and you need to open it right away because everything has a short deadline. I think they now have a portal where info can either be down or up loaded. It depends on the state. You will need to create an account for access to this.
