You can't If the person is not cognizant they can not change any documents.
If you want siblings input on any medical decisions you can ask their opinion and you can make decisions that way.
BUT having more than 1 person make decisions can be a legal nightmare. What happens if you 1 one thing done and the others disagree? For example if mom had signed a POLST saying she did not want Tube Feeding or Intubation and the doctor said that they would like to insert a feeding tube or that they have to intubate. As POA you would follow her wishes and refuse the treatment. What if a sibling wants mom to have the intubation or feeding tube done? Are you ready to fight a legal battle? Or do you cave and mom is now hooked to machines that she will probably never come off of?
I you are referring to your Mom in your profile (88) with dementia, then it is too late for her to add anyone unless she is at the very beginning of dementia and only has mild memory loss and cognitive issues. If you're not sure, take her to an elder law attorney, who will privately interview her to assess if she still has capacity. FYI legal capacity is a pretty low bar.
