Father-in-law has dementia spends most of his time in bed, but gets up to eat and bathes daily, and is changed throughout the day and night, so we believe his chaffing and rash is a product of his absorbent underwear. We have tried various products to remedy the problem the rash continues, though we pay special attention to bathing the affected area(s).

Would appreciate any/all suggestions and or recommendations

Not going to make any recommendation because of lawsuits won against powder manufacturer, causing Ca in women using baby powder.
But check this out-the old-timers may recommend corn starch to keep moisture away. The new-timers may recommend some kind of essential oils.

Yes, I'd try to figure out if it's irritation like diaper rash or a fungal infection, since a fungal infection will need special ointment to get rid of it. I might also try changing the underwear even more often. Sometimes meds make the urine very strong. Regardless of the cause, keeping it dry will be imperative.
