Find Care & Housing
We had it drawn up about 5 years ago when Mom insisted on taking me with her to an attorney to have a new will made. Between the will, the POA, the medical POA, title searches, etc., it all totaled about $400.

If your Mother has dementia and has been clinically diagnosed, I am not sure that she would be considered of "sound mind" to give you PoA...she has to agree to this. You may have to seek guardianship.
Check with a good elder attorney in your state. If he gives you the okay, you can find PoA forms online for your state. Pay attention to the specifics, many forms need to be notarized or witnessed. I used the Legacy Writer's website.
good luck
