We had it drawn up about 5 years ago when Mom insisted on taking me with her to an attorney to have a new will made. Between the will, the POA, the medical POA, title searches, etc., it all totaled about $400.
If your Mother has dementia and has been clinically diagnosed, I am not sure that she would be considered of "sound mind" to give you PoA...she has to agree to this. You may have to seek guardianship. Check with a good elder attorney in your state. If he gives you the okay, you can find PoA forms online for your state.
