The nursing home is saying they closed her account so I can't get the routing or checking numbers. My mom has no ID and is very disabled. Please help!

Find Care & Housing
Visit the Social Security office. You can make yourself her Representative Payee with letters from her doctor saying that she cannot manage her own social security check. Do know, if you do this, that you may have to give Social Security a yearly accounting of money spent out of this check. If you are her POA for financial already you should be able to open a Trust Account into which the check can go without becoming the payee. You also need a letter from the Nursing home as to when she was discharged and should try to have proof of her current address (any mail sent to her there will help.). When you go to Social Security I myself was advised to put in my Brother's SS number if I am there to discuss his case. It's a pain to wait there and you can wait on the phone first if you like. I had a 40 minute wait when I called Social Security, but they were kind, patient, and took time when I got through.

I would call or even visit your local Social Security Office and tell them your mother has been discharged from the NH but the facility is still receiving checks for her care that they are not providing to her. Make sure you have her discharge papers on hand to prove she was discharged.
