How does a living will work with chronic heart failure?

My grandma was just diagnosed with CHF on the left side. I understand it will eventually be both sides failing. She has a living will and I was wondering how CHF works with that. She's 87 so will welcome the end when it comes but what happens as it progresses?

What are the contents of her Living Will? Does it require comfort measures only in some situations?
