Look in your phone book under State Agencies. Ombudsman for your state should be listed. Don't think there is a certain person assigned to a certain SNF.

Each facility, including ALFs and Memory Cares, have the first and last name of the individual acting as the Ombudsman for that particular facility on the wall in public view, with his/her phone number & contact information. It's the law.

Inside the facility itself, there is a contact information sheet posted somewhere *usually in the lobby* with the Ombudsman's name and number. Or else call the SNF directly and ask the receptionist for the information you seek.
