How can I connect with a social worker for help with Aging Aunt?

I would start with contacting Area Agency on Aging in your aunt's county. The county website should have the phone number and information about it.

You can call her doctor for a referral or if she's in a nursing home they'll have a social worker on staff. If your aunt has to go to the ER you can request a social worker there as well.
