Are you collecting Social Security yet? Even though Medicare is at 65, Social Security to get paid 100% is 67. You don't have to wait till 67 to get SS. You can start getting it at 62. But there is a reduction of earnings if you get before 67.

Are you working? You can still collect SS but can only make so much a year.

" In 2025, this limit is $23,400; you'll receive $1 less in benefits for every $2 you earn above this amount. Once you reach your FRA (Full Retirement Age) the limits are removed"

Like said if collecting SS that premium comes out of your payment. Call Medicare and see what you qualify for. My nephew was born with a disability, he is on Medicaid and very low income and receives help. I asked the question "why does my nephew get help with his Medicare premium"

"Medicare Savings Programs (MSPs)

These are state-run programs that help people with Medicare pay for their premiums, deductibles, and other costs. He could qualify for an MSP if his income and assets are below specific levels.

There are different types of MSPs, each with different levels of assistance: 

Qualified Medicare Beneficiary (QMB) Program: Pays for Medicare Part A and Part B premiums, deductibles, and copayments.

Specified Low-Income Medicare Beneficiary (SLMB) Program: Pays for Part B premiums.

Qualifying Individual (QI) Program: Also pays for Part B premiums, but with a slightly higher income limit than the SLMB program.

Qualified Disabled and Working Individuals (QDWI) Program: Helps pay the Part A premium for certain disabled people who returned to work.

Check with SHIP in your state, they have counselors that can assist your if you are low income: https://www.shiphelp.org/
There is something called QMB sponsored by Medicare and Medicaid that you may qualify for :https://www.cms.gov/medicare/medicaid-coordination/about/qualified-medicare-beneficiary-program
SHIP can help you with your application.

Your Medicare premiums should be automatically deducted from your Social Security check each month.
