Jojo this is common with people with Alzheimer's. In Pakistan do you have Memory Care facilities? In this country most would look to move the person to a facility when care can no longer be managed at home. Another suggestion take away his underwear so the only option for him is a incontinence product. It is obvious that grandma needs relief, find it.

Jojo, learn everything you can about Alzheimer's/Dementia which would give you a better understanding why your Grandfather is doing what he is doing. Go to https://www.agingcare.com/Alzheimers-Dementia and scroll down to the articles.

I know there are nursing facilities over in your part of the world, maybe it is time for your Grandfather to move to one to get the higher skilled care that he needs and for your Grandmother to get some relief. If this is not an option because your grandparents refuse to move or there is a waiting list, hire a companion/aide to help with Grandpa.

This sounds miserable and is beyond too much for one caregiver to handle. One person cannot provide 24 hr supervision to someone like this, and I hope there are alternative placements or additional caregivers available and she does not feel bad about using them. There are some incontinence products that look more like underwear that might be an option, and there are styles of clothing that would be more difficult for him to undo by himself but even if you solve the poop problem, the wandering makes it unsafe to have any lapse in supervision. So sorry this is happening to you all!
