Try to involve a third party - a trusted freind of hers or a pastor, priest or Rabbi, perhaps, to talk to her. Often someone outside of the family can lend more authority to something like this. Then tell her, together, that she needs more help and that you've arranged for care for her close to home where you can visit. It won't be easy, which you know or you wouldn't have asked. But if you are at the breaking point, you need to do something.

Some people do this gradually by starting out with adult day care, but you may be beyond that point. If not, it's an option.
