Where can you get an example of a caregiver agreement?

I don't know, but, I would caution you from relying on legal documents from any source than an attorney who knows the facts of your case. It's not always a one size fits all. If funds are an issue, I'd try to pursue help from a legal clinic or legal aid agency.
