I'm not a medical  person, so these are just some guesses and possible questions to ask.

How long has he been on dialysis?   When did the swelling start?

I can't help thinking he's being overly infused, i.e., getting too much fluid, but I confess again that I'm not a medical person and don't know how the fluid is regulated.

Poor guy; that must be so frightening.

I would help as much as you can in relaying the conditions to the doctor who prescribed dialysis, but let your friend get the answers as the doc won't be able to give you information w/o a medical authorization on file, and I think this needs immediate attention.

Good luck, and please let us know how this works out.

This is a medical question that must be put to the doctor in charge of this patient's dialysis. It is not something I have seen. I looked it up and found the following:
https://pubmed.ncbi.nlm.nih.gov/11887776/
This small paragraph mentions some things this syndrome could be due to, but only this patient's doctor can do a workup to track this down.

He really needs to talk to his dialysis team about it, they can help him to understand what is normal and what might be done about it.
