Has she been in the hospital, now in transitional care (rehab), and soon to need a placement? Is it clear that she cannot return to her home, even with some in-home help?
Can she make her own health decisions? For example, if you come up with some options for her can she tell the social worker and discharge planner what she wants? Does she have relatives to help with these decisions? Do you have any authority to speak for her -- for example, does she name you in her healthcare directive as some one to act for her? Will the transitional care place include you in her care conferences?
If she is on Medi-Cal at least the financial end should not be a huge hurdle. Staff at the transitional place should be able to direct you to suitable placements that accept that program. If you have some suggestions you can visit them and provide input for your friend.
Often the kidneys are infected when the diabetes are not well managed. I have T2 diabetes and keep my blood glucose below 100mg/dl by eating a 'low carb high fat' diet. You can also Google diabetesforum where lots of wonderful knowledgeable people offer good free support and help. Many people on the forum start off with some complication and find very useful help on this forum. By being on this diet, I have cut out all 'whites', flour, breads, potatoes, pasta, most fruits, sugar etc. I have tremendous energy now and feel better than I have done in a long time.
