You could also either do an online search of podiatrists affiliated with hospitals in your area, or call the hospitals physician referral services. Sometimes information about medical practices might include whether or not the home care is provided.
There's a remote possibility that the Area Agency on Aging has lists of physicians who do provide home service as well.
Another option is to call local AL, IL or rehab facilities and ask if they have a referral list of home care doctors - rehab facilities at least have arrangements with certain doctors to visit the facility and provide services. Those doctors might also make home visits.
