Where to find an apartment for sister who has beginning stage of dementia?

Can you help me with this? She is staying with me now but want to get an apartment for her with some help with medications and meals for her!

A person with dementia really cannot live alone beyond the very first stage. The kind of apartment that might serve her well is Assisted Living.
