In our area the hospice must have a contract with the N/H in order to be allowed to care for a resident.
As windy said the Dr must authorize the admision but anyone can ask hospice to do an evaluation

The patients treating doctor must authorize hospice care. Hospice usually start with the doc thinks there is 6 months or less of survival. Most nursing homes are familiar with various hospice organizations. Hospice can be administered at home, hospital or nursing home and it's usually covered by Medicare. I am a hospice volunteer and I can attest as to how valuable this service is to patients and their family's. BTW, sometimes patients improve while under hospice care and are removed and can be retuned to hospice at a later date.
