Have not received any information, the representative at the hospital advised me I would receive a call a call, email or a message.

What do your discharge papers say. Does it list the in home service who will be calling you? With a phone #. If u were released Friday or over the weekend the service may not have gotten your information yet. Wait till tomorrow. If they don't call u by early afternoon then call discharge at the hospital.

Call the hospital to confirm that they have the correct phone number or e-mail for you.

Assume nothing, because I am not assuming anything either.

The saying goes like this: "The phone works both ways."
Are you able to contact St. Mary's?

If you were discharged from any hospital over the weekend, wait until Monday to hear back or call them yourself to follow up.

Hoping it was not urgent, such as meeting the IV Nurse for antibiotics when you got home. (As just one example).

If you need in-home PT (Physical Therapy) it is likely that you will need to wait.

You can always make an appearance at the hospital's emergency department if you need immediate care.

If it was St Mary's in Paddington, then the home-from-hospital team should have picked this up and should have been in touch.

7 days a week, 7am-10pm are standard hours for handling hospital discharges in the UK. That's the only reason I asked about location.

St Mary's Hospital roughly where?
What sort of services are you in need of?
Whom did the hospital tell you to contact, exactly?

This is just an internet forum for people who are interested in the subject of aging, whether older themselves or looking after others who are older. The services you see advertised in the margins are also run by AgingCare.com; but if you are waiting for them to contact you you'll need to click on their sections and access them that way.

It doesn't seem likely that a hospital would refer you to AgingCare.com? - but perhaps its fame has spread farther and more widely than I realised! Deservedly so, of course :)

If the hospital told you that you will recieve home care services and you have not, call the hospital.
