My County Office of Aging provides a bus for Seniors.

Many areas have free public transportation.
In the area I live you have to register and the bus or van will come to the house and pick up. the ride has to be arranged in advance.
My area also has a Senior Volunteer group that will also pick up Seniors. Again the ride must be arranged in advance and must be within a specific area.
Check the Senior Service Center in his area.

If you contact his county social services about transportation you will need to ask what their policy is regarding people with cognitive impairment. In my county they will not take an unaccompanied citizen who is impaired for obvious reasons: liability. They are not prepared to keep track of forgetful or vulnerable people. You can consider hiring a companion aid who takes him on errands once a week (and he pays for it). I did this for my 2 very elderly Aunts and it worked great for them. The aid took them wherever they wanted to go.

Contact your Department of Transportation, this is who offers rides for the elderly and disabled.

I have found they are very helpful, you fill out an application and they get you set up for services.
