If this is Medicaid, yes the specialist has to except it and many don't. Medicaid does not pay in a timely manner. Some won't take Medicare.

Be aware that not all specialists take Medicaid. They are not required by law to do so either…

Yes; depending on the insurer, a referral may be needed. You'd have to check with each plan (see what's available at www.marylandhealthconnection.gov/assets/MCO-Comparison-Chart.pdf).
