The nurse came in September and did the health assessment. They said it would take up to 90 days to get my gift card. I called the beginning of December and the lady said she would expedite the card and I would get it in 7-10 days. Didn't get it and can't find their phone number. Thanks.

I have Aetna Medicare. Why are you calling when all of the steps are done on the insurance website?
I go on my site, find the rewards link and click on the healthy home visit. I plug in the date of the visit which is linked to Signify Health.
I also get more savings by clicking on other services I completed such as getting a flu shot. At the end of the year, I click on the link and choose what gift card I want and check out. My card came in 5 days.

You really should frequently check on your insurance website to see what your rewards are missing. It is also suggested that when you get medical bills to look at your provider claims to confirm overcharges.
