What is the purpose of the letter?

If you are employed and are taking a FMLA leave, yes, you may need to submit documentation.

If you are trying to get paid through a governmental agency, you would need a referral for a needs assessment.

If self-pay, the documentation would not be required.

Are you asking how to request payment for care provided to a parent specifically a caregiver construct?

If so hang in there, someone will reply soon re caregiver contracts.
