My mom would reach out to her diaper at night and scratch her butt cheeks ( my English not that great, I hope it’s understood) and upper legs at night and when I wake up and go to her room I can’t sometimes enter due to the smell. While she is scratching, poor thing she will also grab stool and end up smearing it on herself, hands, hair, etc. if I wake my mom up to change her during the night, she screams and won’t let me. She also screams in the morning when I try to get her up to clean her, but I have no option but to get her up to get cleaned.

For the itching, I’m using all the good quality creams, and also making sure she is dry before I put on her diaper, but I can’t figure out how to prevent her from reaching out to her diaper and stool at night. There were times I pulled a stool fragment outside of her mouth, I panicked and took her to the emergency room thinking she will get an infection (emergency staff were a bit annoyed at me). My mom used to be the cleanest human out there and it hurts me to see her come down to. Do adult onesies work? Are there ones specific to summer months? Please help

The same way you deal with the screaming in the morning you deal with it at night. If she needs to be changed you need to change her.
I would check my Husband every 2 hours. He would go to bed at 7PM and I would check him at about 9 or 10 then again about midnight then 3 or 4 and I would get him up between 6:45 and 7 AM. And I would change him sleeping or not. Yes he would wake up but then he would fall back to sleep. (pat on the back here for myself he never had skin issues)
There are adult onsies.
You can also try PJ's with a top and pants and either put a snap or Velcro to join the two so it is more difficult for her to get between the 2 pieces.
