Call your local agency on aging and see what they recommend. If you are wheelchair bound, there are medical transport services that can wheel you into the van while you are in the wheelchair. My Mom had that and it was surprisingly comfortable.
Contact the public transit organizations in your area and ask about point to point transport. If they have, the smaller buses will pick you up at your door and take you to your destination. There may be some mileage limitations though so ask about that.
