Mom does not have much control when getting up and down with her bladder. She sits for long periods watching tv - falling asleep for hours at a time. When she does get up -- there are times that the throw away pads and the washable pads are wet or soaked. The wheelchair pad is another problem as well. I cannot keep it clean. Who can help figure out how to help her? Again the urogynocologist recommended Gemtesa - and that pill does not seem to be doing anything. I cannot breathe when I go to her house to visit - until I take the soiled clothes basket out to the garage and throw away the soiled pads that are in the garbage and take out the bags of used assurance underwear. She is 85 and wants to sit and watch tv and sleep a lot.

If she is refusing to go to the bathroom more frequently, you have a cognitive issue. My mother would also flood her depends and the bed pad in the mornings because she also stayed up very late and sleep very late in her senior apartment. The stink in her place was terrible and she could not keep up with the laundry or change her habits. Things have improved tremendously now that she is in assisted living because they help her get up and dressed in the morning and her schedule is more normal. They also do her laundry and help her bathe.

Your mom is being stubborn and you can’t fix that. If she wants to stay independent, she’ll need to make some changes. Otherwise, you’ll need to get her to pay for daily caregivers to get her on a toileting schedule and clean laundry and messed. You insist that she wants to age in place, but we don’t always get what we want. Especially with cognitive decline.

Has she actually lost control or is it she holds it too long. Holding it will cause UTIs. She needs to completely void. That may mean sitting on the toilet for a little longer until the urge comes on again.

Your Mom needs to try and go on the toilet every two hours. She needs to drink water. When she is checked for a UTI you need to have a culture done to see what bacteria is causing the problem so the correct antibiotic is used. If bad she needs IV in the hospital. To keep her UTI free, D Mannose has been recommended here. My Mom was on cranberry tablets and a probiotic to keep hers from coming back.

Usually, UTIs in the elderly show no signs other than they can have Dementia like symtoms. So Mom having pain is unusual. Part of her continence problem maybe the UTIs. I would ask her doctor about the pain. There maybe something else going on.

You are seeing a urologist specializing in this, so I suspect you have your answer. The urinary system is one of the most complicated in the body being under the control of both the parasympathetic and the sympathetic systems. There is not always an answer so you often come down to being left with finding the best supplies you are able. I wish you best of luck. There's no reason not to try a second opinion, but surgery is unlikely to be an answer.

Ask about Botox injections into the bladder. My urologist has recommended them for female patients in their 80s - but none of the ones I know have dementia. That could change things. The urologist does this in his office and the patient is mildly sedated.

Vaginal estrogen doesn't always help. I've been on it for over a year with no noticeable change at all.

Hire in home help to assist mom with changing her disposable briefs. And with any other help she needs at 85. There isn't an answer for old age in the form of a pill, especially for incontinence.

Sadly it may be time to have your mom placed in a nursing facility where she will get her diaper changed every 2 hours or thereabouts so she won't have to sit in a soaked diaper and pads for hours at a time, as that is a bad UTI waiting to happen.
Most if not all incontinence medications really don't help at all. You can talk to your moms urologist about having a supra pubic catheter(permanent catheter) placed too. That way she or someone will only have to empty her catheter bag about twice a day.
There are no easy answers here but incontinence is often the straw that breaks the camels back and ends up having a loved one placed in the appropriate facility.

Straw. Yes. Facility no. She wants to age at home in her home. I will look into getting some weekly help so I don’t end up in a facility

No, you can not "fix" her incontinence. But you can manage it.

- She needs to be in adult briefs (diapers).
- Hire an aide to come in and change her diapers, and dispose of them outside, as well as washing soiled clothing.
- Buy washable as well as disposable underpads. When you say you can't keep the wheelchair pad clean, is that a seat cushion ? Make sure there's a clean disposable pad on the wheelchair at all times.
- I double up on the pads. Moisture still seeps through, so I place one on top of another.
- She needs to be moved more often and cleaned and have a diaper change several times a day. Sitting for long periods in the same position, and in wetness will start to break down her skin, if it hasn't already. Have you seen her behind?

If hiring someone to come to the home is out of the question, then it would be a good time for your mother to be transferred to a skilled nursing facility. The incontinence care Must be managed properly by someone!

Yes - I have seen her behind - as I worry about that issue for her. She has also been to the dermatologist regularly for skin checks that and all over. All reports are always good. She wants to age at home. She wants to sit and watch tv and eat what she wants when she wants. She is still very sharp. I cannot make her get up when she does not want to. I cannot make her go to bed at night - as she likes to watch movies until about 3 am then head to bed and sleep until the afternoon. She changes herself - even though it is hard and a pain. I do all the washing drying and folding of the pads and clothes as well as all of her grocery shopping. She is capable of getting up and doing some things in her wheelchair but living alone I guess makes those things not important. I am looking into getting someone to come in a few times a week to help out. The UTI’s are painful and I cannot seem to get her to understand why she keeps getting them. We go back to the urogyno this week.
