Are there any adjustments to hourly rates for in-home care. I am doing it for 6 hours a day and have increased from 2 days to 3. Should I expect an adjustment to hourly rate if I increase days/Hours per day.

No, the hourly rate should be the same if just increasing the days. Now, as said, your wife's care needs have changed, then the rate maybe higher.

So you are in NJ, caring for your wife and being compensated by perhaps a gov entity or long term care Insurance or VA?

nj yes. care is out of pocket with an agency right now. looking to lowering our costs - either as i asked or hiring a private person i presently do not medical help and just started partial day and partial week I have been caretaker full time and just started getting part time help.
