Arelis how old are you? Why did care for your grandparents become your job?

My suggestion is to contact the Area Agency on Aging there in Puerto Rico. I imagine they operate similarly to the AAA here in the states since PR is a territory. The AAA will perform a needs assessment on your grandparents.
